The template is adapted from Package Form and Design by Natsumi Akabane.
The background papers are available free for download from Shabby Princess. I used Paisley and Dots from the Happy Go Lucky collection.

How to assemble the bunny box
Print designs on paper, front and back, if desired.
Cut out box. Crease all fold lines.
Glue together box at side and top.
The bottom flaps don't need glue, but you can glue them if you like.
Pull each ear apart a little along the center cut to give them shape.
